City Permit Parking forms are available here but to obtain your permit, you must bring them into the Salt Lake City Transportation Office at 349 South 200 East, Suite 450. Internet renewal is not available and permits are not able to be issued via US Mail. If you have any questions, please contact our office at 801.535.6630
