Below are links to a Conflict of Interest Disclosure
Statement that must be completed by each nominee, including nominees for
reappointment. The first section of the form pertains to employment.
The second section pertains to self-employment and disclosure of partial
interest in any other business entity. (Once
you are a board or commission member, if your financial circumstances change,
another Conflict of Interest Disclosure Statement must be filled out annually
and submitted to the Mayor’s Office by January 1st).
Please note: The
Conflict of Interest Disclosure Statement MUST BE NOTARIZED.
If you do not have access to a Notary Public, one is available in the
Salt Lake City Recorder’s Office, 451 South State, Room 415, from 7:30 am to
5:00 pm, Monday through Friday (you must present a driver’s license or picture
identification).
Originals of both the Conflict of Interest Disclosure
Statement and the Application must be returned to the Mayor’s Office, 451
South State, Room 306, in order for your application to be processed.
