Interactive Grant Form Instructions

  Important - Read this first!

All City Arts Grants interactive forms are available as a PDF file that you can view, download and print. You will need the free Acrobat Reader from Adobe to access the form. Acrobat Reader will allow you to fill out the form on your computer, but when you close the file, you will lose any information you have entered. We suggest that you complete one page at a time on your computer and then print it.

If you do not have Adobe Acrobat Reader on your computer, click here or on the icon below to download the free application. (If you have a version earlier than 6.0, please download the most current version.)

1. Click on the link in the table below to open the application form file.

2. When the file opens, download the file by saving a copy to your hard drive before you type in any information. (You still will not be able to save any changes you make to the file, but you will avoid delays using your connection to the Internet.) Click the icon shown below to save the file. Make sure you click the icon on the Acrobat toolbar, not your browser's toolbar.
   
   

3. The form is protected so you can enter information only where required. The narrative space is also limited to the number of characters in the space provided. You can cut and paste text from a word processing program into the form, but if there is more text than the space allows for, that text will not show up on the form. Use the TAB key to move between fields on the form.

4. When you come to option buttons or checkboxes, TAB to the selection you want and press ENTER to mark the box.
